Ensures the safe and effective use of medications by providing medication order verification while assessing for optimal drug product selection, dosage regimen design, effective administration and drug therapy monitoring services in the UVA Medical Center. Monitors inpatient and outpatient drug therapies when appropriate, and works with relating medical staff to find alternatives when needed, to ensure the best care possible is being given.

The Charles O. Strickler Transplant Center at the University of Virginia Health System is the only comprehensive transplant program in Virginia with active programs in adult liver, kidney, pancreas, heart, lung, and islet cell transplantation, as well as pediatric kidney and heart transplantation. The center performs approximately 150 new transplants each year and manages over 2000 transplant recipients who account for 4000 transplant clinic visits annually (http://www.healthsystem.virginia.edu/internet/transplant/). The transplant clinical pharmacy team consists of the clinical pharmacists and the pharmacy clinical coordinator. The clinical pharmacists rotate providing comprehensive clinical pharmacy services as fully integrated members of the interdisciplinary transplant team in the inpatient and ambulatory setting for both abdominal and thoracic transplant services. Clinical pharmacists monitor drug therapy, actively participate in interdisciplinary health care rounds, provide pharmacokinetic and therapeutic consults, patient education, report ADRs, perform MUEs and quality reviews, participate in emergency response, and in the precepting of pharmacy students, PGY1-pharmacy and PGY2-specialty residents, including the PGY2 transplant pharmacy resident.
