The Population Health Pharmacist will work as part of the Enterprise Care Management Office for the BayCare Health System and leverage their pharmacy expertise to manage care for a defined population. The pharmacist will partner with the disease management team, transition of care nurses, and care coordinators to improve patient medication adherence, and optimize medication therapy. The pharmacist will also collaborate with physicians to meet medication related CMS quality measures. Experience with quality improvement initiatives to lower costs and improve patient outcomes is desired. This individual will have an opportunity to precept and mentor pharmacy residents and students. Excellent communication skills including a strong knowledge base and experience or residency training in ambulatory care are required.
Current Board Certification in Ambulatory Care or Pharmacotherapy is preferred
Experience in Managed Care or Population Health preferred.
