The Medication Safety Pharmacist-Pediatrics will be responsible for developing and implementing initiatives directed at improving the safe use of medications and medication use systems for pediatric patients across the Cleveland Clinic Health System. Principal duties and responsibilities include:
Leading process improvement efforts designed to reduce medication errors or improve patient safety.
Serving as an internal consultant on medication safety, safety improvement methodologies and medication accreditation compliance strategies.
Managing data reporting and information related to medication safety.
Assisting with implementing a culture of shared individual and system accountability (e.g., Just Culture).
Identifying and pursuing opportunities for applied research in the area of medication safety.
Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems.
Recommends formulary alternatives when appropriate.
Compounds and dispenses medications for delivery to patient care areas.
Provides drug information and education to medical staff.
Communicates with physicians and other clinicians as necessary to facilitate care.
Other duties as assigned.
PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy.
Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current State within 90 days of hire.
Complexity of Work:
Requires critical thinking skills, decisive judgment and the ability to work unsupervised or with minimal supervision.
Must be able to work in a stressful environment and take appropriate action.
Internal candidates: Minimum of six months as a Pharmacist II
External candidates: Minimum of two years experience in hospital pharmacy OR have successfully completed a PGY1 Residency.
