Clinical Pharmacist – Riddle Hospital (Media, PA)

- PGY2 experience and with a background in Infectious Disease
- ASHP accredited residency training in infectious disease or equivalent training/experience is required.
The Clinical Pharmacist will assist in implementing and sustaining ongoing services related to clinical drug use and QA services of the Pharmacy Department and in coordinating ongoing development and restructuring of the professional practice of pharmacy within MLHS by defining and conducting educational programs for professional staff. Candidate will also aid in assessing needs across the system and involve necessary persons in total patient care concepts as pertinent to optimization of drug use, interpret and evaluate medication orders for patients of MLHS and consult with necessary healthcare professionals as necessary to optimize drug therapy, routinely participate in patient care rounds, either with other healthcare providers or independently, and other pharmacy related projects, support the healthcare team by providing drug information and guidance related to appropriate and cost effective drug therapy and participate in various committees throughout the system.
Qualified candidate must have a PharmD Degree from an accredited College of Pharmacy (clinical clerkships, residencies and specialties in hospital-oriented environments preferred), current registration as a Registered Pharmacist in PA with good standing with the PA Board of Pharmacy and have a minimum of 2 years of hospital pharmacy experience with a PharmD Degree. Recent graduates and Pharmacists eligible for reciprocity will be considered as long as licensure is complete before the end of the 90-day probationary period. Hospital Pharmacy experience includes IV admixtures, automated distribution functions and clinical counseling. Drug information newsletter article, P&T drug monograph, formulary class review, clerkship preceptorship, drug information lecture or other equivalent project. Work history with good to excellent record in organization, accuracy, quality and quantity of work and attendance is required.
