Seeking an Ambulatory Care Pharmacist in Kingman, Arizona!
Kingman Regional Medical Center is looking for an ambulatory care specialist to join the Disease Management Clinic (DMC) team. The DMC is a pharmacist run clinic providing disease state medication management services through a Collaborative Practice Agreement with our community practitioners.
The pharmacist will focus on anticoagulation management as part of expanded services within the DMC. Responsibilities will include providing direct patient care, assisting with implementation of DAWN AC Anticoagulation Management software, and participating in continuous improvement of protocols.
The pharmacist also will also be provided with opportunities that include:
- Serving as a preceptor for APPE students and PGY1 residents
- Completing a teaching certificate or other identified learning opportunities
- Presenting to the community through KRMC’s healthcare topic seminar series
- Participating in Diabetes Self-Management Education classes and Cardiac Rehab classes
