Position Description: The Clinical Pharmacist Specialist in Internal Medicine position is a challenging, diverse, and dynamic position with opportunities and responsibilities in inpatient pharmacotherapy management, teaching, and mentoring. Our 20-member Pharmacotherapy Team of 10 highly skilled Pharmacotherapists each having completed at least two years of post-graduate training, and 10 PGY-1 and PGY-2 pharmacy residents, strives to provide high caliber and comprehensive pharmaceutical care services. The 318-bed acute care hospital provides medical and surgical services in our acute and intensive care units. Other services include: a step-down cardiac care telemetry unit, a 30-bed ventilator unit, acute rehabilitation, 2 acute psychiatric stabilization units, and a 911 receiving emergency department.
The responsibilities of this position are to provide medication therapy management and pharmacotherapy consultations and for admitted patients in the medical/surgical care units and coordinate with care managers to provide transitions of care services including medication reconciliation/histories and discharge education to high-risk patients. The Clinical Pharmacist will participate in medical attending teaching rounds, participate in codes, participate on pharmacotherapy consultation services, provide pharmacokinetic dosing, approve restricted antibiotics, and provide transitions of care services. Furthermore, the Clinical Pharmacist plays a vital role in several hospital committees such as the pain management, anticoagulation, diabetes, and research committees. Our pharmacotherapy consultation services include Pharmacokinetics, Antimicrobial Stewardship, Clinical Pharmacology, and Adverse Drug Events.
A responsibility of this position is to serve as a faculty member, role model, mentor and preceptor for our ASHP-accredited Pharmacy Residency Programs in Critical Care, Internal Medicine, Geriatrics and Pharmacy Practice. There will be a non-compensatory faculty appointment at the Clinical Assistant/Associate Professor of Pharmacy Practice rank with Touro College of Pharmacy in Manhattan. Opportunities to precept Pharm.D. students also exist. This position offers an excellent opportunity to fulfill leadership, learning, teaching, research and publishing endeavors.
