FAIRVIEW HOSPITAL founded in 1892, is a faith-based community hospital with 517 licensed beds. It is fully accredited by Joint Commission, with a certified Level II trauma center. The Manager, Clinical Pharmacy Services is responsible for the following: *Leadership and mentoring of approximately 15 direct reports *Assists the director to develop clinical programs at Lutheran Hospital (a 204-bed specialty hospital) *Assume leadership as the RPD for the PGY1 residency starting with the next residency calendar year (July, 2018) *Develop standard operating procedures to achieve patient outcomes accountabilities *Measure and report success to targeted metrics *Creates a continuous quality improvement goal within their team, which continues to strive to move services to value *Committee involvement to optimize patient care and standardization at the local and system level *Develop programs that optimize the costs of pharmaceutical care through pharmacists review *Maintains a collaborative climate that values a highly engaged work-force *Provide assistance to nursing regarding collaboration on patient care initiatives *Coordinates communication of system formulary updates at Fairview and Lutheran Hospitals *Assists in maintaining of policies and procedures that provide guidance for physicians, nurses and pharmacists on the delivery of pharmaceutical care.
