Responsible for providing the prospective evaluation and monitoring of drug therapy and make patient specific recommendations to improve pharmacotherapy outcomes for patients with specific disease states, or patients assigned to a Primary Care Provider within Penn State. Provide education regarding drug therapy to health care professionals, patients and caregivers. ADDITIONAL INFORMATION: - Resource will be located in ASB
- Will review patient charts based off of patient lists showing noncompliance with medication adherence
- Will partner with physician, and work with patient to address barriers (via telephone, or portal message).
- Some opportunity for face-to-face review with patient if practice site has space.
- Will recommend to physician alternative medications based on collaboration with payors and best practices.
- Will require documentation with in the EMR and communication with care team
- Must be comfortable working collaboratively as part of a care team
MINIMUM QUALIFICATIONS: - Bachelors of Science degree in Pharmacy or Doctorate Degree in Pharmacy required.
- ASHP-Accredited PGY-1 Pharmacy residency or 3 years of relevant clinical pharmacy experience required.
- Active and valid PA licensure or eligibility as a Pharmacist required.
KNOWLEDGE, SKILLS, & ABILITIES: - Ability to prioritize and organize daily activities to meet patient care needs required.
- Effective verbal communication skills, time management skills and ability to manage multiple priorities required.
- Ability to analyze clinical data and perform calculations.
- Computer and Microsoft Office skills required.
- Extensive knowledge of pharmacotherapy required.
- Attention to detail and critical thinking skills required.
- Effective mentoring and precepting abilities required.
