The ideal candidate will provide pharmaceutical care services in assigned clinic areas. In the hospital setting, provides accurate interpretation and dispensing of medication orders and clinical involvement with providers, allied health staff and patients regarding medication therapy decisions. Provides guidance to the technical staff in the department as well as learning experiences for introductory and advanced pharmacy practice students as well as pharmacy residents. In the clinic setting, the pharmacist is part of an interdisciplinary care team providing comprehensive medication management services. Designs, implements, monitors, evaluates and modifies patient pharmacotherapy to ensure that medication therapy is indicated, effective, safe, and convenient.
Completes documentation and billing processes for pharmacist activities and participates in collaborative practice and/or protocol-driven practice as appropriate. This individual will participate in educational and scholarly activities and educates healthcare professionals, students and patients regarding rational drug therapy. Participates in quality improvement and resource management initiatives.
Must be a Graduate of an accredited Bachelor's or Doctor of Pharmacy Program or nearing graduation. License to practice Pharmacy in Wisconsin. Prior experience through an internship or employment as a Pharmacist preferred. Demonstrates excellent organizational skills. Able to tolerate high stress levels and frequent interruptions. Utilizes mature, independent judgment.
