The mission of our Pharmacy teams is to administer excellent pharmaceutical care to NYC Health + Hospitals Patients. Through the responsible and efficient provision of drug therapy and positive patient outcomes overall quality care is achieved. We are currently seeking to various Ambulatory Care Clinical Pharmacists for opportunities within New York City.
Under the direction of a Director of Pharmacy and collaborating physician(s), these Pharmacists will be responsible for drug management therapy of patients with specified disease states in the ambulatory clinic setting. Ambulatory Care Clinical Pharmacists adjust or manage a drug regimen of a patient, pursuant to a patient specific written order or protocol made by the patient's physician. They also provide educational expertise and support to other departments within the hospital when required to meet the goals of the organization.
PRINCIPAL DUTIES & RESPONSIBILITIES
Enter into collaborative practice agreement with physician(s)/ physician group(s) and provide direct Collaborative Drug Therapy Management.
Maintain liaison relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission Standards.
Assess medication regimens and make modifications so as to achieve desired therapeutic outcomes; provide direct pharmaceutical care in the outpatient ambulatory setting.
Provide drug information to patients, physicians, nurses, and pharmacists.
Participate in educational programming for patients.
Participate in staff development and education for pharmacy, medical and nursing staff, including, but not limited to, noon conferences, grand rounds, in-services, and competency programs.
Document interventions, adverse drug reactions (ADRs) and medication errors.
Participates in the preceptorship of students during their clinical pharmacy education experiences; as applicable, participates in the supervision of pharmacy residents completing an ASHP-accredited pharmacy practice residency program.
Perform other related duties
PharmD Degree Required
Possession of a valid NY State license to practice Pharmacy
Completion of a pharmacy post graduate residency, PGY-1 or PGY-2, or
Eligible for Pharmacy Board Certification or currently Pharmacy Board Certified
