Founded in 1921, The Portland Clinic has a well-earned reputation for providing extraordinary patient care in a private multispecialty outpatient clinic setting. We are seeking a collaborative part-time (.6 FTE, 3 days/week) Clinical Pharmacist to join two others in the department. This position may transition to full-time in the future.
The Clinical Pharmacist will work closely with our primary/specialty care teams on a daily basis with a focus on medication quality, safety and utilization; patient education/service and medication use initiative issues. The Clinic Pharmacist will assist in managing select patient populations or disease states with “complex” medication-related issues.
Education Licenses Required:
Doctor of Pharmacy from an accredited school of pharmacy.
Licensure by the Oregon State Board of Pharmacy.
Pharmacy residency or two years of experience in an ambulatory setting.
Prior work with an interdisciplinary team including physicians.
Experience in evaluating data, interpreting findings, and communicating recommendations to target audiences including physicians, pharmacists, nurses and patients.
