As a member of the Digital Medicine Care Team, the Digital Medicine Clinical Pharmacist will leverage digital tools and patient-entered data to optimize the management of medication therapy to meet chronic disease goals and improve patient’s health outcomes. The Digital Medicine Clinical Pharmacist will work directly with patients via telephonic outreach and electronic communications to identify, resolve and prevent medication related problems, as well as meet treatment goals. Along with the Digital Medicine Health Coach, the Digital Medicine Clinical Pharmacist will promote healthy lifestyle and behavior changes. Working under a collaborative practice agreement, the Digital Medicine Clinical Pharmacist will work independently to combat clinical inertia, optimize medication therapy, and provide patient education. The Digital Medicine Clinical Pharmacist will assess medication therapy for effectiveness, safety, tolerability, and address adherence issues to maximize patient satisfaction and meet clinical goals. The Digital Medicine Clinical Pharmacist will provide ongoing monitoring of medication therapy to optimize clinical outcomes and will also identify and address gaps in care.

Review patient entered data and modify medication therapy to meet clinical goals
Perform regular telephonic outreach to patients to communicate changes and assess progress
Monitor labs and other pertinent information to ensure patient safety
Communicate with referring physicians as needed regarding patient progress
Provides patient education about medication therapy and disease state management
Educates staff, students, patients, health care providers and other health care professionals.
Mentors residents, students and new staff to meet departmental goals.
Assumes responsibility for professional development and continued education.
Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.

Required – Graduate of an accredited School/College of Pharmacy, PGY1 Residency in Pharmacy Practice
Preferred – PGY2 Residency in Pharmacy Practice
Required – Licensed to practice pharmacy in the State of Louisiana, or eligible for licensure by examination or reciprocity from another state.
Knowledge Skills and Abilities (KSAs)
Must have computer skills and dexterity required for data entry and retrieval of information.
Effective verbal and written communication skills and the ability to present information clearly and professionally.
Must be proficient with Windows-style applications, various software packages specific to role and keyboard.
Strong interpersonal skills.
Ability to exhibit good clinical experience in specialty as well as leadership/teaching skills.
Ability to understand the distributive process of the Pharmacy within 45 days from hire.
Ability to travel to offsite locations as required.
Ability to work flexible schedule as needed such as nights, evenings, weekends, etc.
