Position description: Wexner Medical Center at The Ohio State University is recruiting a highly motivated Pharmacist to expand Ambulatory Care services within the health system. The pharmacist should function as a highly independent practitioner to improve patient outcomes through performing patient centered assessment and medication management, in-depth medication and disease-state education. The candidate may work in a variety of clinics including interdisciplinary settings as well as pharmacist-run clinics to provide quality ambulatory care to all patients, including a high volume of direct patient care. Services may include, but are not limited to: Anticoagulation Management Service, Primary Care Clinics (University Health Connection or Central Ohio Primary Care), Inflammatory Bowel Disease, Nephrology/Rheumatology, HIV, or Neurology.
Beyond direct patient care, the pharmacist will assume leadership roles in identifying evidence-based opportunities for improvement in patient care and organizational processes, participate in appropriate hospital and/or departmental committees relevant to practice area, and act as a resource to provide answers to drug information questions. This position is also responsible for supervising Pharm.D. students, pharmacy residents, and providing educational programs for interdisciplinary staff (physicians, nurses and pharmacy). Must effectively navigate and utilize the electronic medical record and other technology to perform job.
Description of institutions: Wexner Medical Center at The Ohio State University consists of University Hospitals, The Arthur G. James Cancer Hospital and Richard J. Solove Research Institute, University Hospitals East, The Richard M. Ross Heart Hospital, several outreach facilities, and Central Ohio Primary Care. The Department of Pharmacy is comprised of over 300 employees including pharmacy administrators, clinical specialists, pharmacists, pharmacy technicians, secretarial/clerical personnel, and pharmacy residents.
Required or desired credentials: Graduate of an accredited school of pharmacy, eligible for licensure as a Registered Pharmacist in the State of Ohio. PGY-2 Ambulatory, Pharmacotherapy, or Internal Medicine residency training is strongly preferred.
