UF Health Jacksonville (UFHJ) is a 695 bed academic medical center affiliated with the University of Florida. The hospital offers a Level I Trauma Center, Level III NICU, proton beam facility, Poison Center, certified stroke program and many other programs. We also have a large Ambulatory Care Center with outpatient pharmacists practicing in a variety of clinics. Inpatient pharmacists at UFHJ have a unit-based practice with a focus on patient care and teaching incorporated into our practice model. Epic is our electronic health record used in our inpatient and outpatient areas. Our pharmacists precept 6 PGY1 residents and 6 PGY2 residents in Ambulatory Care, Critical Care, Emergency Medicine, Infectious Diseases, Medication-Use Policy and Safety, and Pain Management. Our team of pharmacists consists of diverse, passionate, and collaborative individuals. The department practices an interdisciplinary approach and maintains an excellent relationship with physicians, nurses, case managers, and other disciplines.
We are currently recruiting for a full-time Primary Care Clinical Pharmacist position.
The successful candidate will serve as a preceptor for our ASHP accredited PGY1 and PGY2 residency programs and for students from the University of Florida and surrounding colleges. There are opportunities for research, publishing, didactic lectures and improvement of patient safety and professional practice through hospital committees. The department supports professional development and encourages participation in the regional/national professional organizations. Other responsibilities include, but not limited to:
Provide comprehensive medication therapy management to patients through a collaborative practice
Serve as a specialist on drugs and their utilization therapy
Participate in a patient-focused, team-based multi-disciplinary model of patient centered care to optimize patient outcomes through direct and indirect interactions with patients and providers
Design a cost effective, therapeutic regimen based on patient-specific characteristics
Monitor and assess laboratories/tests as indicated within scope of practice
Monitor and assess therapeutic and adverse effects of drug therapy through evaluation of physical and laboratory parameters
Serve as a preceptor to pharmacy students and residents
Participate on various pharmacy committees.
Dispense medications in the ambulatory pharmacy in compliance with state and federal drug laws as regulated by the State Board of Pharmacy, the DEA, and FDA
