The Mercy Health-St Vincent Medical Center (MHSVMC) PGY2 Ambulatory Care resident will be given the opportunity to develop clinical expertise through the Mercy Health Medication Management Service. Practice settings for pharmacist visits include primary care office, hospital, patient home, outpatient pharmacy, and telephone. Expected resident activities throughout the year will include medication management, care transitions, chronic disease state management, patient education, population health, precepting APPE students, research project, leadership, and Lean Six Sigma principles. System level initiatives are also available for DM, COPD, CHF, opioids and specialty pharmacy.
