Community Health Network in Indianapolis has excellent opportunities at multiple locations for experienced Ambulatory Care Pharmacists who have a collaborative approach to patient care. These positions are located throughout Indianapolis, IN as well as Kokomo, IN and Anderson, IN. Selected candidates will join our successful team of more than 30 ambulatory care pharmacists. These current positions are located within primary care, cardiology, women’s health, behavioral health, and infectious disease practices where the pharmacist will work closely with physicians and healthcare teams to optimize medication regimens for patients with a variety of conditions.
Adjust medication regimens for patients utilizing a collaborative practice agreement, complete Medicare annual wellness visits, transitions of care visits, and population health activities based on clinic setting.
Provide extensive patient education regarding disease states and medications, with a particular focus on patients who are at high risk for hospitalization or poor health outcomes.
Work with patients to overcome medication adherence issues.
Use various strategies to lower patient medication expenses.
Answer medication therapy questions and serve as drug information resource for physicians and nursing staff.
Graduate of accredited college of pharmacy
Valid Indiana Pharmacist license; Board Certification expected within 2 years of working in this position
Completion of PGY1 residency; completion of PGY2 residency with ambulatory care focus is strongly preferred
Strong clinical, leadership, interpersonal and communication skills
Ability to interpret and prepare detailed reports, as well as analyze information to formulate appropriate patient care plans
