Provides Leadership, vision, and strategic planning for medication safety across the health system
Establishes medication management standards through literature review, benchmarking, and collaboration.
Collaborates with administrative staff, risk management, nurses, physicians, IT, and others to develop and maintain safe medication practices.
Responsible for conducting medication safety assessments, developing/reviewing guidelines and policies, chairing the medication safety committee, and for providing education and consultation regarding safe medication practices.
Monitors, review, and synthesizes recommendations from reported adverse drug events and trends.
Participates in RCA, FMEA, and quality reporting activities
Participates in the review of relevant technologies and processes
Participates in drug diversion activities
Pharmacist Licensure in the State of Ohio (Eligible)
PGY 1 Residency or 3-5 years of hospital pharmacy experience
PGY 2 Residency in Medication Safety (Preferred)
Strong clinical pharmacy education
Excellent written and oral communication skills
