Our PGY2 Ambulatory Care Pharmacy Residency is a one-year postgraduate specialty training program that offers residents an immersive experience in all aspects related to ambulatory care pharmacy after they have completed a PGY1 residency.
The goals of the program are to help the resident specialize in patient care related to ambulatory patients, gaining experience in collaborative interdisciplinary ambulatory practice. Residents will be able to manage medication therapy for complex patients with multiple disease states. Additionally, residents will gain experience in ambulatory pharmacy leadership, including management and precepting activities.
As a pharmacy department that offers comprehensive health services for the community, our program offers PGY2 ambulatory care experiences in diverse settings (please see website for full listing). During the 12-month program, PGY2 ambulatory care pharmacy residents will complete training in pharmacy operations and rotate through learning experiences that see a variety of disease states. The resident will also complete a year-long residency project to be presented at Western States.
At the completion of the PGY2 Ambulatory Care Pharmacy Residency, the resident will be fully prepared to serve as an ambulatory care pharmacist, with the potential to become a supervisor or manager.
PharmD degree from an ACPE-accredited school of pharmacy
Completion of an ASHP-accredited PGY1 pharmacy residency program
Eligible for licensure in the state of California
Minimum of 240 hours of experience as an outpatient staff pharmacist preferred
Favorable recommendation letters from clinical faculty, preceptors, and/or former employers
Demonstrate collegiality, clinical knowledge, interest in research, and motivation necessary for successful completion of residency training
Demonstrate desire to pursue a career in ambulatory care pharmacy
Completion of on-site interview with residency leadership, pharmacy supervisors, and preceptors
Additional Salary Information: $59,000-62,000 per year
About County of Santa Clara Health System Pharmacy Residency Program
County of Santa Clara Health System includes a Hospitals and Clinics delivery system, as well as Department of Public Health. The pharmacy residency program is primarily conducted at Santa Clara Valley Medical Center Hospital & Clinics which is located in San Jose, CA. There are multiple ambulatory care clinics and clinic-based pharmacies located throughout the County, from Milpitas to Gilroy, and East Valley to Sunnyvale.
We have a large, successful residency program including 13 PGY1 residents and 5 PGY2 residents. Our PGY2 programs are:
