The Clinical Pharmacist - Specialist assumes responsibility for medication management for a core group of patients. The Clinical Pharmacist - Specialist will be a motivated individual interested in providing clinical pharmacy services for a core group of cancer patients (specific therapeutic area) in inpatient and ambulatory setting. In collaboration with the medical team, the responsibilities of the Clinical Pharmacist - Specialist will include optimizing drug therapy, therapeutic drug monitoring, prospectively evaluating drug therapy (especially antineoplastic therapy) for appropriate indication, dosage, drug interactions, adverse dug events and allergies, chemotherapy and other medications orders review, provision of drug information, and patient counseling. The Clinical Pharmacist - Specialist will also act as a liaison between pharmacy and medical staff to educate health professionals regarding current drug-related procedures, policies, guidelines, and treatment pathways; participate in quality assurance programs to enhance pharmaceutical care. They will be also responsible for developing and implementing pharmacy programs and initiatives. There will be ample teaching opportunities available such as precepting PGY2 oncology specialty residents and pharmacy students and providing educational lectures for medical and pharmacy staff. This position requires some involvement in clinical research.
License to practice as a Registered Pharmacist in the State of New York or eligible for New York state pharmacist license
Education and Experience
1. Doctor of Pharmacy degree; or
2. Master’s degree and the equivalent of one (1) year of full-time clinical pharmacy or pharmacy-related experience; or
3. Bachelor’s degree and the equivalent of three (3) years of full-time clinical pharmacy or pharmacy-related experience.
Required degrees must have been granted by an accredited college or university or one recognized by Roswell Park Comprehensive Cancer Center as following acceptable educational practices.
Incumbents must be credentialed as a member of the organization Medical Staff (Allied Health Professional) within four months of date of hire as a condition of continued employment.
The preferred candidate will be board certified in oncology, will have PGY 2 Pharmacy Residency Training, and will have previous experience in a hospital pharmacy.
