Job Description - PHARMACY MEDICATION SAFETY COORDINATOR - WEST REGION - PHARMACY
The Pharmacy Medication Safety Coordinator shall support the mission of the hospitals and pharmacy department by preventing, identifying, and resolving drug related problems to assure patient safety, improve patient satisfaction, and promote medication safety initiatives. In addition to these duties, the Pharmacy Medication Safety Coordinator shall be responsible for the design and implementation of safety initiatives and team member education/development.
Required Education and Experience:
One to two years of experience focused on improving medication related systems and processes
Excellent communication and leadership skills to influence care providers at all levels of the organization
Knowledge of general principles of medication use safety, system theory, culture of safety, medication error reporting systems, human factors engineering and performance improvement
Preferred Education, Experience & Certifications:
PGY2 Medication Safety residency training preferred; 2-3 years in medication safety role acceptable in lieu of residency
Experience in the adult/pediatric medication use process with 2-3 years as a Clinical Pharmacist
Direct work experience and proven results in the following areas: patient safety, RCA, detailed event reviews, ACA, FMEA, clinical process improvement, or healthcare quality improvement
ASHP or equivalent Professional Certificate Program
