The PGY2 Infectious Diseases Pharmacy Residency Program is a 12-month program within the County of Santa Clara Health System. The program has three main focuses: 1) develop strong antimicrobial stewards; 2) provide exposure to a diverse population with various infectious diseases and antimicrobial needs; 3) emphasize growth of strong preceptorship skills.

Required Learning Experiences:
¨ Antimicrobial Stewardship Program Service (2 core rotations, 1 primary preceptor focused)
¨ Antimicrobial Stewardship or Infectious Diseases Consult at O’Connor or St. Louise Regional Hospitals
¨ Infectious Diseases at Stanford Hospital
PharmD degree from an ACPE-accredited school of pharmacy
Completion of an ASHP-accredited PGY1 pharmacy residency program or one in the ASHP accreditation process (i.e. – Candidate Status)
Eligible for licensure in the state of California
Favorable recommendation letters from clinical facility and/or former employers
Demonstrate collegiality, clinical knowledge, interest in research, and motivation necessary for successful completion of residency training
Demonstrate desire to pursue a career in critical care pharmacy
Completion of on-site interview with residency program coordinator, residency program director, director of pharmacy services, and preceptors
Additional Salary Information: $59,000-62,000 + overtime for weekend staffing
About County of Santa Clara Health System Pharmacy Residency Program
County of Santa Clara Health System includes a Hospitals and Clinics delivery system, as well as Department of Public Health. The pharmacy residency program is primarily conducted at Santa Clara Valley Medical Center Hospital & Clinics which is located in San Jose, CA. There are multiple ambulatory care clinics and clinic-based pharmacies located throughout the County, from Milpitas to Gilroy, and East Valley to Sunnyvale.
We have a large, successful residency program including 13 PGY1 residents and 5 PGY2 residents. Our PGY2 programs are:

- PGY2 Health-System Pharmacy Administration & Leadership
- PGY2 Infectious Diseases
