University Health System (UHS) is a nationally recognized teaching hospital and network of outpatient healthcare centers. UHS has a growing hematology/oncology service for inpatient pediatrics, adolescents and adults in addition to a dedicated outpatient infusion center We are looking for a highly motivated individual with excellent teamwork skills to join our growing pharmacy team and expand our clinical pharmacy services for hematology/oncology patients!
The pharmacist will primarily support the outpatient infusion center and provide direct patient care services and comprehensive pharmacotherapy management for our adult hematology/oncology patients. The pharmacist will recommend, monitor and evaluate chemotherapy regimens to optimize drug therapy and improve clinical outcomes. The pharmacist will provide patient education and chemotherapy counseling in addition to education and in-services for the multidisciplinary health care team.
Additional Expectations of this position are as follows:
Have expert knowledge in antineoplastic IV admixture preparation including USP 797 & USP 800.
Review and refine process for timely and safe antineoplastic preparation and delivery.
Help develop policy and procedures and establish clinical pharmacy services for patients in the outpatient infusion center.
Work with multi-disciplinary teams to ensure guideline and pathway compliance.
Improve hematology/oncology services through active participation in quality, safety and cost-savings initiatives, Clinical Management Team meetings, and P&T Committee meetings.
Actively participate in pharmacy intern and residency training programs.
Advance oncology pharmacy services through research, education of healthcare professionals, and professional organizational involvement.
Pharmacy licensure in the state of Texas and a B.S. Pharm or Pharm. D. degree
Completion of an ASHP-accredited PGY2 Pharmacy Residency in Hematology/Oncology or equivalent experience (PGY1 plus 3 additional years of experience, or 5 years of experience, in health-system pharmacy)
Experience in a hospital-based, outpatient infusion center
Board Certification in Oncology Pharmacy
