Prepares and dispenses drug orders per physician request according to established policies, procedures, and protocols.
Supervises and directs pharmacy support personnel.
Maintains competency required for current job title/position.
May serve as a lead pharmacist in a specialty area.
Monitors drug therapy regimens.
Assists with pharmacy consult service per hospital protocol.
Detects and reports adverse drug reactions.
Sustains the hospital drug formulary.
Provides clinical consultation and clarification to providers.
Provides drug information education to patients.
Conducts designated interventions as defined in department such as IV to PO conversion, renal dosing, pharmacokinetic dosing, and anticoagulation dosing and monitoring.
Participates in antibiotic stewardship activities.
Contributes to the advancement of the pharmacy department and medication management of the organization.
Supports the mission of CHRISTUS Health.
Graduate of an ACPE accredited school of pharmacy with a Doctor of Pharmacy degree;
Prefer one of the following: BPS board certification, residency, or fellowship;
Pharmacist license and registration in Texas.
