The Ambulatory Care Clinical Pharmacist works closely with providers to deliver population-based disease management, patient and provider education and collaborative drug therapy management via telehealth. The pharmacist actively manages medication therapy, completes medication reviews and collaborates with providers to optimize medication therapy, improve medication adherence, promote wellness, prevent disease, and improve outcomes. This position will support Banner Health Network providers and members.
Requires a degree in Pharmacy from an accredited College of Pharmacy. Requires PGY2 residency or equivalent experience.
Requires current pharmacy licensure in the state of practice within 90 days of hire.
Board certification by Board of Pharmaceutical Specialties.
Additional related education and/or experience preferred.
