The Medication Safety Officer works closely with the Pharmacy Leadership and other Institute leaders to ensure best practice medication safety processes are routinely followed in all areas of Pharmacy Services and throughout Roswell Park. The Medication Safety Officer stays up-to-date with new standards, guidelines, and best-practices in medication safety, actively pursues to improve knowledge and understanding of medication safety through regular and ongoing staff development efforts particularly directed at pharmacy, medical and nursing staff, and develops and presents department level and institute level proposals for medication safety related changes. They also implement, manage projects, monitor, measure and present outcomes for these medication safety changes initiatives, continually monitor performance of the Institute from a medication safety perspective, and assume responsibility for tracking, trending and presenting of medication error information and adverse drug reaction information. The Medication Safety Officer also leads the Institute’s multidisciplinary Medication Safety Committee, participates actively in other related Institute committees, investigates, responds to and actively participates in patient safety incidents involving poor medication safety practices, supervises and leads personnel and projects assigned to the medication safety team, responds to higher level medication safety requests, develops policies and procedures, and consults on institution policies. They perform drug utilization review, develop treatment algorithm/protocol, implement educational programs, and act as a liaison between pharmacy, medical staff and nursing on all aspects of medication safety. This position requires collaboration with other hospitals, groups such as ADCC, and with related professional organizations such as ISMP, ASHP, and MSOS.
License to practice as a Registered Pharmacist in the State of New York
Education and Experience
1. Doctor of Pharmacy degree and the equivalent of two (2) years of full-time clinical pharmacy or pharmacy-related experience; or
2.Master’s degree and the equivalent of four (4) years of full-time clinical pharmacy or pharmacy-related experience; or
3.Bachelor’s degree and the equivalent of six (6) years of full-time clinical pharmacy or pharmacy-related experience.
1. Incumbents must obtain licensure as a Registered Pharmacist in the State of New York within 90 days of hire as a condition of continued employment.
2. Incumbents must be credentialed as a member of the Roswell Park Medical Staff (Allied Health Professional) within four (4) months of date of hire as a condition of continued employment.
The preferred candidate will have project management experience and process improvement experience. They will also be proficient in Microsoft Office, EMR, and Patient Information Systems. They will also have good analytical skills.
