The Norton Healthcare Post-Graduate Year-2 (PGY2) Ambulatory Care Pharmacy Residency Program is a one year ASHP-accredited residency designed to train pharmacists who specialize in the provision of ambulatory care services at diverse practice settings within a health system. Norton Healthcare is an integrated community health-system in Louisville, KY with four adult hospitals, one pediatric hospital, greater than 90 physician practices, four on-site community pharmacies, and one specialty pharmacy. In addition to developing clinical skills to manage complex chronic disease states in conjunction with an interdisciplinary team, the resident will also be involved in developing ambulatory clinical pharmacy services and precepting students and PGY1 residents. This program offers direct patient care learning experiences in primary care in a Patient Centered Medical Home, diabetes management/MTM for Norton Healthcare employees, transitions of care, ambulatory oncology, neurology in a multiple sclerosis practice, specialty pharmacy, and community pharmacy. Some learning experiences are delivered in a longitudinal design while others are provided in a more traditional block format. Longitudinal learning experiences are also offered in project management and leadership.
