IU Health is recruiting for two Clinical Pharmacy Specialists—Ambulatory Care. One specific to the heart failure population and the other specific to the pulmonary hypertension population. Summary: Heart Failure: IU Health is recruiting a clinical pharmacy specialist to enhance our services to the heart failure patient population in the Indianapolis area. The successful candidate is highly motivated, resourceful, and has outstanding leadership and communication skills. Pharmacists are an integral aspect of patient care at IU Health, with a strong presence and excellent relationships with provider and nursing partners across all specialties. Our existing Ambulatory Clinical Specialists provide care in a variety of areas, including primary care, cardiology, HIV, HCV, hematology/oncology, solid organ transplant, pulmonary hypertension, and population health. This ambulatory specialist will be responsible for patient management which may include: heart failure across the spectrum of severity, tobacco cessation, hypertension, anticoagulation/direct oral anticoagulants in consultation with nurses/cardiologists, immunizations, and transitions of care. In addition, pharmacists are involved in direct efforts of system-wide population health services to improve clinical outcomes and reduce hospitalizations for at-risk patient populations. The ideal candidate has experience in establishing and sustaining pharmacist services in the ambulatory setting with an applied knowledge of collaborative drug therapy management, documentation, and billing issues. Summary: Pulmonary Hypertension: IU Health is recruiting a clinical pharmacy specialist to continue services in our Pulmonary Hypertension Association-Accredited Center of Comprehensive Care. The successful candidate is highly motivated, resourceful, and has outstanding leadership and communication skills. The Academic Health Center in Indianapolis provides nationally recognized clinical care to patients from across Indiana and beyond with severe, complex illnesses. The Advanced Heart and Lung Care Center on the Methodist Hospital campus is the largest PH center in the state and the only one to carry the Center of Comprehensive Care accreditation. Our center currently manages 600 active patients, with approximately 140 new patient referrals annually. Pharmacists are an integral aspect of patient care at IU Health, with a strong presence and excellent relationships with provider and nursing partners across all specialties. Our existing Ambulatory Clinical Specialists provide care in a variety of areas, including primary care, cardiology, HIV, HCV, hematology/oncology, solid organ transplant, and population health. This ambulatory specialist will be accountable for patient management including patient education, management of side effects, and protocol development and maintenance. The ideal candidate has experience in establishing and sustaining pharmacist services in the ambulatory setting with an applied knowledge of collaborative drug therapy management, coordination/transitions of care, and medication access. Responsibilities include: - providing clinical pharmacy services in PH clinic that supports patients and providers
- medication and adverse drug event management (REMS, therapeutic transition recommendations, prostacyclin titrations)
- medication education including demonstration of prostacyclin devices
