The Clinical Pharmacist Practitioner (CPP) would be responsible for Chronic Care Management of patients from the provided by the practice. This position will provide direct patient care within the physician practice. The CPP will collaborate with the patient’s primary care provider to optimize evidence based pharmaceutical care. The CPP will evaluate the patient for drug related problems and will use an EMR to review patient’s conditions, laboratory test results, current medication list and pharmacy claims data. The pharmacist will communicate discrepancies and drug related problems found and resolutions to the discrepancy or problem with the provider within the EMR. The CPP will develop appropriate care plans and follow up for patients based on individualized patient needs. Care is delivered within the context of a long-term health care partnership with the patient that emphasizes health improvement, wellness, and disease prevention.
The Clinical Pharmacist Practitioner will deliver excellent customer service utilizing Relationship Based Care to advocate for safety, patient preferences, and optimal communication through partnering with patients/families and colleagues. Ideal candidates for this position should possess excellent verbal and written communication skills. The CPP must have the ability to work collaboratively as a member on an interdisciplinary team, establish and maintain effective working relationships with other healthcare professionals, practitioners, patients and families.

Current active license to practice pharmacy in North Carolina
Having the following Clinical Pharmacist Practitioner requirements:
1. has earned Certification from the Board of Pharmaceutical Specialties, is a Certified Geriatric Pharmacist as certified by the Commission for Certification in Geriatric Pharmacy,
2. has completed an American Society of Health System Pharmacists (ASHP) accredited residency program with two years of Clinical Experience approved by the Boards;
3. holds the academic degree of Doctor of Pharmacy, has three years of Clinical Experience approved by the Boards, and has completed a North Carolina Center for Pharmaceutical Care (NCCPC) or American Council on Pharmaceutical Education (ACPE) approved certificate program in the area of practice covered by the CPP Agreement;
4. holds the academic degree of Bachelor of Science in Pharmacy, has five years of Clinical Experience approved by the Boards, and has completed two NCCPC or ACPE approved certificate programs with at least one program in the area of practice covered by the CPP Agreement.
