Position Summary: Inova Health System is seeking a highly motivated Ambulatory Clinical Pharmacy Specialist with a focus in Infectious Disease/HIV to establish a clinical practice with the Inova Juniper Program. Successful candidates will collaborate with ambulatory clinic physicians/providers to provide comprehensive medication and education plans for patients seen within the clinic setting while offering unsurpassed health care experiences to our patients, healthcare providers, and community. This position would also assist in developing outpatient antimicrobial stewardship initiatives and education across the Inova system.
The major work activities of this position include, but are not limited to:
Collaborate with physicians, nurses, and other allied health providers to provide comprehensive medication and education plans for patients seen within the Juniper clinics.
Ensuring safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols; providing accurate, adequate, and timely drug information; providing staff education to physicians, nurses, and pharmacists; responsible for supporting quality initiatives through data analysis and management; and supporting effective operation through policy development, staff engagement, and expeditious medication order verification and delivery.
Provides oversight and assists with the development and implementation of evidence-based medication guidelines/protocols.
Supports quality initiatives through data analysis and management.
Develops criteria for appropriate use of medications.
Participates in Pharmacy and Therapeutic Committee (P&T) activities, as appropriate. Evaluates medications for formulary additions.
Provides clinical interventions and counseling.
Enroll or assist in insurance adjudication and monitor billing procedures when appropriate to ensure the best care possible is being given.
Demonstrate ownership for the welfare of the patient by managing transitions of care for patients from the acute to ambulatory care setting.
Serve as a mentor, preceptor, and/or residency program director (if applicable) for pharmacy interns, students, and residents.
Work with health plans, other insurers, pharmacy benefit managers, employer groups, government, and other third-party programs to coordinate specialty pharmacy and associated disease management services.
Required Degree(s): Pharm.D.
Desired Certification(s): BCIDP or BCPS-AQ ID, or BCACP, AAHIVP (American Academy of HIV Pharmacist)
Experience Desired: ASHP Accredited PGY2 Infectious Disease or Ambulatory Care (with ID experience) or equivalent work experience
