This job maintains and ensures complete and appropriate drug therapy in a multidisciplinary pharmaceutical care environment. Assumes responsibility for the management of pharmacy services with the goal of ensuring the patient receives complete and appropriate drug therapy and improving patient outcomes. Provides drug information to patients and healthcare providers.. Performs drug utilization review and checks for appropriateness. Works with Specialty Fulfillment team to ensure complete and timely receipt of medications and compliance with drug therapy.
Performs Patient Assessments, manages drug therapy, and provides patient counseling to optimize patient care
Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
Detect, monitor, and mitigate adverse drug reactions.
Communicate with patients, providers, and caregivers
Assumes responsibility for professional development and continued education
Maintains documentation relative to the job
Maintains and fosters and environment of teamwork and engagement
Participates in special projects/committees as needed
Required – Doctor of Pharmacy (PharmD), Completion of a PGY1 Pharmacy Residency or equivalent experience
Preferred - Specialty pharmacy experience
Oncology Preferred – Completion of PGY2 Oncology Residency
Required – License to practice Pharmacy by the Louisiana Board of Pharmacy
Preferred – BCPS
Preferred for Oncology Pharmacist: BCOP
Preferred for HIV Pharmacist: AAHIVP
Knowledge Skills and Abilities (KSAs)
Must have computer skills and dexterity required for data entry and retrieval of patient information.
Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
Must be proficient with Windows-style applications, various software packages specific to role and keyboard.
Strong interpersonal skills.
Proficient in Microsoft Office.
Other related duties as required.
