This position will support the work of the Intermountain Telehealth program, as part of an Infectious Diseases (ID) Telehealth team that includes a second ID Clinical Pharmacist. The Clinical Pharmacist will contribute to comprehensive pharmaceutical care and support for patients and healthcare staff at 17 Intermountain small community hospitals and help expand regional telehealth programs with important outreach activities for contracted or affiliated healthcare systems. Responsibilities within Intermountain hospitals include reviewing VigiLanz alerts, providing answers to drug information questions, developing protocols, managing antimicrobial stewardship projects and initiatives, contributing to system-wide antimicrobial stewardship initiatives, in support of the ID Telehealth Service. Responsibilities at outreach telehealth sites include establishing new antimicrobial stewardship programs (which will include travel to outreach sites), providing answers to drug information questions, managing antimicrobial stewardship projects and initiatives, and supporting the ID Telehealth Service. The Clinical Pharmacist will serve as a member of the Intermountain Antimicrobial Stewardship Committee and System ID Pharmacist Team. Additional responsibilities include participation in departmental and interdisciplinary committees to improve patient safety, therapeutic outcomes, and cost savings. The clinical pharmacist is expected to serve as a medication therapy expert and to advise physicians, pharmacists, nurses, and patients about pharmacotherapy. Opportunities for collaborative clinical research are available and encouraged. The Clinical Pharmacist will provide a clinical rotation to precept pharmacy students, pharmacy interns, and pharmacy residents, including an PGY-2 ID Pharmacy Resident.

Review antimicrobial-related alerts for small community hospitals using VigiLanz. Contact pharmacists and physicians to make stewardship interventions. Support front-line pharmacists in responding to VigiLanz alerts. Approve restricted antimicrobials.
Support Intermountain’s telemedicine expansion efforts for infectious diseases and antimicrobial stewardship at outreach hospitals.
Provides accurate drug information to other health professionals and patients to assure safe and appropriate use of medications. Serves on committees as a resource on drugs and/or pharmacy policies as required.
Participates in the multidisciplinary team to provide feasible therapeutic recommendations and alternatives that improve patient drug therapy. This may include drug, dosage form, dose, route, dosage interval and duration. Monitors physician orders to ensure safe, correct, and appropriate use of medications. Monitors, documents and evaluates patient response, drug effects, and adverse medication events.
Actively participates in department cost savings initiatives. Participates in organization surveys (Joint Commission or other applicable regulatory agencies) or audit inspections.
Participates and supports the training programs and mentorship of students and residents.
Is knowledgeable and compliant with State and Federal laws, regulations, pharmacy policies, procedures and standard operating procedures, formulary and pertinent Clinical Practice Guidelines.
Provides professional, engaging customer service to all patients, and healthcare providers with whom they work.
Current license as a Pharmacist in Utah.
Controlled substance license.
Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy and the distribution of approved drugs, controlled substances, and investigational agents.
Three or more years of experience in providing mentorship, training, or in a preceptor assignment evidenced by meeting the ASHP preceptor requirements.
Prefer one meets one of the following three requirements:
Five or more years of experience working as a pharmacist in a patient focused clinical practice setting
Doctor of Pharmacy degree plus one or more years of post-graduate training in an accredited pharmacy residency program
Doctor of Pharmacy degree plus two or more years of post-graduate training in an accredited pharmacy residency program. (Intermountain verifies both degree attainment and educational institution accreditation following an offer of employment.)
Maintains board certification by the Board of Pharmaceutical Specialties
