We are seeking a Peds Heme/Onc Pharmacy Clinical Specialist to join our Fairfax Department. The Pharmacy Clinical Specialist is responsible for the following:
collaborate with physicians, nurses, and other allied health providers to provide excellent pharmaceutical care and services in the Pediatric Hematology/Oncology specialization
ensuring safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols; providing accurate, adequate, and timely drug information; providing staff education to physicians, nurses, and pharmacists; responsible for supporting quality initiatives through data analysis and management; and supporting effective operation through policy development, staff engagement, and expeditious medication order verification and delivery.
Provides oversight and assists with the development and implementation of evidence-based medication guidelines/protocols.Supports quality initiatives through data analysis and management.
Develops criteria for appropriate use of medications.
Participates in Pharmacy and Therapeutic Committee (P&T) activities, as appropriate. Evaluates medications for formulary additions.
Functions as a therapeutic consultant in areas that may include rational and appropriate use of medications, clinical pharmacology, pharmacokinetics, disease state management, and nutrition.
Provides clinical interventions and counseling.
EDUCATION: Graduate of an ACPE Accredited School of Pharmacy PharmD.
EXPERIENCE: Clinical Pharmacy Practices 1 year of clinical practice 3 years of In-Patient Pharmacy experience to include in a Pediatric Hematology/Oncology setting (PREFERRED) 1 year of experience as a clinical pharmacy specialist
TRAINING: ASHP Approved residency Completion of an ASHP Approved post-graduate Year 1 (PGY-1) Residency
CERTIFICATION: ASHP Certification Program Board Certified Pharmacotherapy Specialist or certification obtained within 2 years of employment.BLS and ACLS (if critical care)
LICENSURE: VA Board Pharmacy Licensure Eligible for licensure
