Job Summary: Reporting to the Director of Formulary Management and Clinical Pharmacy, the Clinical Pharmacy Coordinator – Medication Utilization leads optimization of medication utilization to ensure the safe, clinically effective, and cost-effective medication use across Sutter Health System. This position will oversee programs relating to the utilization of medications to identify and support development of medication use guidelines, policies or protocols regarding optimal medication use. This position supports the Pharmacy and Therapeutics Committee and Subcommittees to further support the formulary process, which includes facilitation and leadership of Subcommittees. The incumbent in this position will collect, analyze and disseminate information on medication utilization initiatives, and integrate with systems and processes including clinical, safety, prescribing, administration, procurement and distribution systems to optimize medication use across Sutter Health System. The incumbent in this position will identify opportunities for stewardship of high cost or high use medications to limit waste, generate revenue, improve quality, and/or reduce overall cost of care. The incumbent executes and supports the implementation of approved action plans and continuously tracks progress of stewardship initiatives. Additional responsibilities may include strategic partnerships with Service Line leadership, representation on medical center committees and collaboration on department and inter-professional initiatives. This position requires participation within a team-based approach to advance key medication use and clinical pharmacy practice programs and initiatives across Sutter Health and serves as a pharmacy lead for key clinical pharmacy program initiatives. Using Lean principals and tolls, provides content expertise and leadership in continuous quality improvement and, as needed, proposal for redesign of existing practices to support the delivery of system-wide standardized, safe and effective medication use practices across the continuum of Sutter Health. Principal Accountabilities: Responsible for the establishment and coordination of Medication Utilization programming at Sutter Health - Develops template for financial tracking of medication utilization initiatives/stewardship.
- Directs data analyst to create spreadsheets.
- Coordinates initial and ongoing internal and external benchmarking of Medication Utilization Initiatives
- Reports ongoing findings/results to System Pharmacy and Therapeutics Committee.
- Collaborates work of Medication Utilization with the FMCP team to include considerations that may become evident upon review of therapeutic classes or new drug technologies within the formulary management process for Sutter Health P & T Committee.
Develops and shares report card process for analysis/facilitation of medication utilization stewardship. - Creates a roll-up report of facility and/or foundation specific compliance and progress with system-wide medication utilization programming.
- Meets periodically with individual DOPs/Clinical Coordinators relevant to progress and assistance with Medication Utilization programming
- Measures the outcome of the report card process.
- Measures medication utilization programs on a rolling 12 month basis for new initiatives with periodic re-checks of initiatives to maintain the gain over time.
- Based on ongoing monitoring results, may make recommendations for modifications to increase the effectiveness of the programs.
- Provides ongoing feedback/results in an ongoing fashion to participants/stakeholders to foster encouragement and positive feedback of continued programming support.
Creates customized initiatives/interventions to support medication utilization programming. - Prioritizes medication utilization initiative in terms of safety/operational challenges/opportunity for quality and cost improvement in collaboration with the Medication Utilization Subcommittee of the System Pharmacy and Therapeutics Committee
- Develops medication utilization initiative program template, including analyses, materials, and clinical support information.
- Assists medical groups and DOP/Clinical Coordinators throughout the system to modify and execute the medication utilization initiatives and programming. Facilitates collaboration and support between facilities throughout the system.
Develops and presents provider, pharmacist and patient education programs to support the Formulary Process/Medication Utilization Programming. - Creates template/format for provider and patient education materials necessary for support of Medication Utilization programming.
- Incorporates and distributes education materials as part of formulary management and medication utilization programming
- Assures that education materials are consistent with approved policy and guidelines.
Serves as a Preceptor to Pharm.D. Students and PGY1/PGY2 residents Additional Requirements: - Travel in Northern California may be required for off-site meetings and for medical group programs and meetings.
