We are recruiting a highly motivated critical care pharmacist for coverage of our cardiothoracic/vascular surgery intensive care areas. The Cleveland Clinic pharmacy services provide an exciting environment for high quality patient care with a complex population. Responsibilities include decentralized coverage of assigned units, coordination of pharmaceutical care incorporating multidisciplinary involvement, therapeutic drug monitoring, minimization of safety issues, ensuring cost effective pharmacotherapy plans, and medication reconciliation. Collaboration with other healthcare professionals is imperative.
Practice Model/Patient Population • Shared decentralized coverage of our cardiothoracic surgery and vascular surgery intensive care units • Managing new admissions, interaction with front line providers for acute and ongoing patient care needs. • Coordinating with other providers to ensure ongoing continuity of care
Additional Responsibilities • Responsibilities for precepting APPE, PGY1 Pharmacy Practice, PGY2 Residents • Expectation to lead or contribute to projects and research and serve as pharmacy representative on appropriate committees

Education: • PharmD or Bachelors of Science degree in Pharmacy from an accredited college of pharmacy and completion of a post-graduate residency or fellowship training required.
Certifications: • Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current State within 90 days of hire. Board Certification preferred.
Work experience • Completion of a post-graduate year 2 residency (PGY2) Critical Care Residency or • Completion of a post-graduate year 1 residency (PGY-1) plus 3 years of experience as a clinical specialist or equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring including 50% or more of time spent in the specialty area or • 5 years of experience as a clinical specialist or equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring including 50% or more of time spent in the specialty area
