ï¿½ Refill, load and unload medications into the Med-Station;

Sterile IV Therapy Production
1. The pharmacist is able to:
ï¿½Assist technicians in performing tasks in the preparation of sterile products;
ï¿½Utilize proper aseptic technique;
ï¿½Recycle product to minimize waste;
ï¿½Prioritize work (i.e. Stats made first); and,
ï¿½Accurately check technician prepared products prior to release.
Maintenance of all required reports and records
1. Able to maintain and update all required files, reports and records as required by law, regulation or department policy.
1. Routinely performs all required safety checks.
2.Thoroughly completes all required documentation.
3.Insures that all staff use required protective measures and techniques.
4.Uses appropriate technique to dispose of chemotherapy waste properly.
Inpatient Medication Dosage Preparation and Dispensing
1.Follows established policies regarding the filling of new medication orders, requests for refills, and missing med requests.
2.Accurately checks support staff accuracy prior to medication release.
3.Able to prioritize work to triage stat, now and new orders in order of importance.
4.Follows through on all new orders.
5.Able to fill requisitions from nursing floors in a timely fashion.
1.Checks cassettes for accuracy if filled by a technician.
2.Identifies duplicate therapies
3.Identifies possible clinically significant drug interactions
4.Identifies and corrects all orders entered incorrectly
5.Completes cassette checking in a timely manner

ï¿½Modifies does appropriately.
ï¿½Writes progress notes per protocol.
5. Drug Information
ï¿½Maintains current on new drugs.
ï¿½Able to answer questions in a timely and accurate manner.
6. Provides continuing education lectures, written reports, in-service Ed lectures, written pharmacy newsletter articles at least once annually.
Participates in organization-wide quality improvement initiatives
1.Medication storage area inspections:
ï¿½Routinely inspects all stocks of medications to ensure the absence of outdated, unusable or mislabeled products.
ï¿½Assures proper storage conditions.
ï¿½Documents all storage areas that are non-compliant and resolves any issues with appropriate nursing staff and informs the Director of any continuing problems.
2.Adverse drug reaction identification, reporting and prevention:
ï¿½Able to appropriately participate in the hospitalï¿½s ADR Reporting Program.
3.Medication error identification, reporting and prevention:
ï¿½Participate effectively in the hospitalï¿½s mediation error identification and reporting program.
ï¿½Acts as a resource to reduce the incidence of medication errors and is pro-active in programs to reduce patient risk of medication misadventures.
4.Identifies areas of cost optimization while maintaining highest quality of services
5.Responsible for reading and understanding all departmental and hospital policies and other pertinent communications.
6.Attends 80% of staff meetings or reads and initials staff meeting minutes.
7.Continuously assesses and improves the performance of care and services provided.
8.Demonstrates knowledge of CQI through application of the principles in the department.

A. Minimum Education: Graduate of a college of pharmacy accredited by the American Council on pharmaceutical education (ACPE). Doctor of pharmacy degree preferred.
B. Minimum Experience: Two years of hospital experience preferred. Hospital pharmacy residency preferred.
C. Preferred Skills and Abilities: Must possess a specialized pharmaceutical knowledge in addition to a good working knowledge of pharmacokinetic dosing and nutritional support. Effective verbal and written communication skills. Computer working knowledge.
Skills and Proficiencies Specific to the Position
1.Possesses the skills and knowledge to provide all aspects of pharmaceutical care for all PEDIATRIC patients, including calculating medication dosages, monitoring the effects of medication and outcomes of therapy;
2.Possesses the skills and knowledge to provide all aspects of pharmaceutical care for all ADOLESCENT patients, including calculating medication dosages, monitoring the effects of medication and outcomes of therapy;
3.Possesses the skills and knowledge to provide all aspects of pharmaceutical care for all GERIATRIC patients, including calculating medication dosages, monitoring the effects of medication and outcomes of therapy;
4.Possesses the skills and knowledge to provide all aspects of pharmaceutical care for all ADULT male and female patients, including calculating medication dosages, monitoring the effects of medication and outcomes of therapy.
