The Ambulatory Care Clinical Pharmacist will provide direct patient care at a designated CHRISTUS Trinity Clinic location as part of a pharmacist-run anticoagulation and primary care clinic. He or she works closely with medical and allied health staff to manage patient pharmacotherapy regimens.The clinical pharmacist will manage anticoagulation therapy per protocol as part of both face-to-face visits as well as telephone encounters. Activities include evaluation and interpretation of lab results, patient education and counseling, and initiation and dose adjustment of anticoagulants in patients receiving short- or long-term anticoagulation, including perioperative anticoagulant management. In addition, he/she will perform drug therapy management for diabetic and hypertensive patients under a collaborative practice agreement as part of population health initiatives. The clinical pharmacist will participate in pharmacy-based projects and multidisciplinary committees, and will precept student pharmacists and pharmacy residents.

Requires a degree in Pharmacy from an accredited College of Pharmacy.
Requires PGY2 residency or equivalent experience.
Requires current pharmacy licensure in the state of practice within 90 days of hire.
Board certification by Board of Pharmacy Specialties.
