About the job:
The Medication Safety Officer supports and performs medication safety analysis, investigations, and other system-wide initiatives related to Pharmacy Services. This position reports directly to the Chief Pharmacy Officer.
What you will do:
Conduct medication error-related root cause analysis; Provide data reporting and analysis as needed to regulatory/accreditation agencies
Develop medication administration policies for organizational consideration and approval and ensure that pharmacy staff use best practice guidelines in the area of medication safety
Participate in integrating medication safety policies with nursing and other clinical areas
Collaborate on developing initiatives to manage high alert, hazardous, and look-alike/sound-alike medications
Serve as a reference on current and developing regulatory requirements and practice standards related to medication safety
All other duties as assigned.

What we are looking for:
Education: Doctor of Pharmacy from an accredited school of pharmacy
Licensure / Certification: Valid State of Ohio license to practice as a Pharmacist
Experience: 3 to 5 years of relevant work experience as a Pharmacist in a hospital setting. Previous experience with medication safety preferred
Awareness of the rationale for and importance of effective medication safety policies.
Proven ability to identify practices and conditions that compromise or pose a risk to medication safety and patient safety.
Ability to make recommendations for system-wide decisions, changes, and improvements regarding medication safety.
Experience mentoring peers on medication safety practices and identifying/handling adverse events
