Yale New Haven Hospital (YNHH) is currently recruiting several Clinical Pharmacist, Multispecialty positions. Pharmacists in this role rotate through multiple clinical areas as well as operations shifts. Different areas of focus including those listed in the posting are possible depending on the candidate's experience, interests, and departmental needs.
Responsibilities: · Participation in multidisciplinary rounds and pharmacy committees · Ensuring accuracy and appropriateness of medication orders via order verification · Ensuring cost-effective medication use · Identifying processes to prevent adverse events and optimize patient outcomes · Educating patients and healthcare professionals · Leading projects in alignment with the pharmacy strategic plan · Precepting pharmacy students and pharmacy residents
This position requires the pharmacist to work rotating day and evening shifts Monday through Friday, every fourth weekend and holiday rotation according to departmental policy.

Located in scenic New England, Yale New Haven Hospital is spread out on 2 campuses within walking distance of each other. New Haven is located on the shoreline of Connecticut (Long Island Sound), a two hour drive from New York City and Boston.
The individual must be a graduate of an accredited College of Pharmacy with a B.S. or Pharm.D. degree and hold a valid CT pharmacist license. Completion of an ASHP accredited PGY1 pharmacy residency, or equivalent years of experience, is required. Successful candidates must have excellent written and oral communication skills.

Additional Salary Information: This position is eligible for advancement through the pharmacist career ladder. Funds available to support travel to professional conferences (some restrictions apply).
