Responsible for coordinating the pharmacy department's efforts related to clinical pharmacy initiatives. Provides support to the Therapeutics Committee in areas related to formulary management, medication use and JCAHO initiatives. Supervises and assesses the activity of the Clinical Staff Pharmacists and plays an active roll in their ongoing development. Responsible for completing drug information request and participates in educational efforts related to medication use. Identifies, organizes, and completes performance improvement projects related to medication use. Coordinates drug therapy for the investigational drug program.
Minimum Requirements: Work requires the level of knowledge acquired through graduation from an accredited College of Pharmacy. PGY-1 pharmacy residency or equivalent experience. Pediatric experience required. One to three (1) to (3) years of recent hospital pharmacist experience required.
Must be able to communicate effectively with physicians and other healthcare professionals concerning Therapeutic Committee decisions and recommendations related to pharmaceutical care. Requires ability to coordinate clinical programs with Clinical Staff Pharmacists. Communication skills must be effective in both oral and written forms. Required to make independent evidence-based decisions related to medication therapy. Must develop new clinical programs as well as contribute to improving existing programs. Must have working knowledge and ability to use with ease, various software programs utilized to aid in both clinical and educational support.
Preferred Qualifications: Pharm.D. or Masters degree preferred. PGY-2 pharmacy residency. 5-10 years hospital pharmacist experience
