What You Will Do
Dell Seton Medical Center at The University of Texas in partnership with Dell Medical School at the University of Texas is seeking a Clinical Pharmacist Specialist in the area of solid organ transplantation to support its new kidney transplant program. This represents a unique opportunity in a rapidly growing market for which the partnership is prepared to commit resources to build a regionally and nationally recognized program. The partnership envisions a stepwise approach to implementation of kidney, liver, and pancreas transplantation over the next few years.
The Clinical Pharmacist Specialist will serve as the primary resource for pharmaceutical care related to transplant. This individual will work closely with members of the muti-disciplinary transplant team to create local protocols and provide comprehensive patient care to transplant candidates, recipients, and donors in the inpatient and outpatient setting. Additionally, (s)he will participate in educating pharmacy and other staff on transplant medications. Other activities may include participation in research and precepting pharmacy students and pharmacy residents. The transplant pharmacist may float to provide coverage of clinical pharmacy needs as required.
As an Associate with Ascension Texas, you will have the opportunity to assure the appropriate selection, dosing, administering, and dispensing of medication orders in the specialized area of solid organ transplants. Serves as a subject matter expert in pharmacy discipline and provides guidance across pharmacies and to clinical leadership.
Develops protocols for medication use in transplant patients
Provides clinical pharmacy support to inpatient transplant service and outpatient transplant clinic.
Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems.
Recommends formulary alternatives when appropriate.
Provides drug information and education to medical staff and patients.
Communicates with physicians and other clinicians as necessary to facilitate care.
Assists with academic functions such as serving as a preceptor for pharmacy students.
Provides patient counseling when required.
What You Will Need
Pharmacist credentialed from the Texas State Board of Pharmacy.
Bachelor's degree in Pharmacy or Doctorate of Pharmacy required Pharmacy residency preferred
PGY-1 Pharmacy Residency Strongly Preferred. PGY-2 Pharmacy Residency in solid organ transplant strongly preferred.
Experience providing pharmaceutical care to solid organ transplant patients strongly preferred.
