A Pharmacy Coordinator is a leadership role designed to provide coordination of work for their respective area of responsibility in order to maximize productivity, fiscal responsibility, quality, and excellent pharmacy outcomes.
This position is for the Medication Safety Coordinator
Major responsibilities of the Medication Safety Coordinator include:
Works collaboratively with leadership, staff, and medical staff to design, implement and assure compliance with a health system medication safety program that meets standards of care for medication safety, accreditation, and federal and state regulatory requirements
Coordinates the activities of the Medication Safety Committee
Leads the development and implementation of the District Medication Error Reduction Plan (MERP)
Participates in adverse drug event (ADE) review, investigation, and data analysis identifying methods to reduce frequency of preventable ADEs
In conjunction with Risk Management and Patient Safety, investigates medication safety-related issues within the organization including participation in applicable Root Cause Analysis and Failure Mode and Effects Analysis
Identifies best practices for medication safety, analyzes current practices that may contribute to medication errors and works collaboratively with the leadership of various disciplines to take proactive steps for error prevention.
Responsible for leading and assuring safe medication practices of the pharmacy department.

CA Pharmacist License
A strong background in clinical pharmacy practice coupled with experience in hospital pharmacy management.

Clinical Inpatient Pharmacy services utilize an integrated decentralized staffing model. Clinical services include pharmacokinetics, anticoagulation management, and automatic intravenous to oral switches, automatic renal dose adjustments and automatic therapeutic interchanges.
