Franciscan Health is seeking a creative, innovative pharmacist to join our ambulatory care pharmacy team. We an have exciting opportunity for an ambulatory care pharmacist, with a focus in cardiology, to practice in a dynamic team-based care model within a Franciscan Physician Network provider office. This position is currently a part-time position.
Within this model, the pharmacist will work under collaborative practice to manage chronic disease states with a focus on heart failure, participate in population health initiatives, and serve as a resource for providers and ancillary staff. The pharmacist will be responsible for aiding in achievement of quality metrics as identified by the health system. Opportunities also exist to precept pharmacy students, PGY1 and PGY2 ambulatory care pharmacy residents.
Requirement for this position are as follows:
Hold a Doctor of Pharmacy (PharmD) from an ACPE-accredited college of pharmacy
Completion of a PGY1 residency with at least 3 years of direct patient care experience and/or PGY2 residency. If the pharmacist has not completed a residency, he/she must be Board certified in Ambulatory Care (BCACP) at time of hire.
Board certification in Ambulatory Care (BCACP). If not currently board certified, the pharmacist must achieve board certification within 24 months of hire, regardless of previous experience.
