The PICU Clinical Pharmacist will work as part of the multidisciplinary team to deliver safe, appropriate, comprehensive, and cost-effective drug therapy. PICU is comprised of 18 beds and includes management of single and multi-organ system failure, trauma, general and subspecialty post-surgical care, and advanced ventilator support.
Job responsibilities include the following:
Participate in multidisciplinary rounds with the healthcare team
Review drug therapies and apply appropriate pharmacokinetic and pharmacodynamic principles to provide optimal drug therapy recommendations
Interview and counsel patients, families and/or caregivers regarding safe and effective medication use
Develop and implement clinical practice guidelines to enhance pharmaceutical care and medication safety
Participate in medication-use evaluations and formulary management
Serve as a drug information resource for providers, nurses, fellow pharmacists, and other healthcare providers
Detect and document adverse drug reactions, clinical interventions, and medication errors
PGY1 and PGY2 pharmacy residencies OR PGY1 residency with 2-4 years of intensive care practice experience (pediatrics preferred) with responsibility for direct clinical patient care
