Children’s Hospital New Orleans is expanding pharmacy services to the ambulatory space with the creation of outpatient and specialty pharmacy services. To support the growth, we are recruiting a Pediatric Ambulatory Clinical Pharmacist to aid in the establishment of Ambulatory Clinical Pharmacy Services.
Job responsibilities include the following:
Assist in creating, implementing, and monitoring the delivery of ambulatory care pharmacy services
Design a comprehensive drug therapy and patient education plan for patient-specific problems
Review drug therapies and apply appropriate pharmacokinetic and pharmacodynamic principles to provide optimal drug therapy recommendations
Pursue Collaborative Drug Therapy Management with providers when appropriate
Develop and implement clinical practice guidelines to enhance pharmaceutical care and medication safety
Collaborate with patients, caregivers, and other healthcare professionals
Coordinate with liaison technician on prior authorization process
Serve as a drug information resource for providers, nurses, fellow pharmacists, and other healthcare providers
Detect and document adverse drug reactions, clinical interventions, and medication errors
