General Description: The Clinical Pharmacist for Hematology/Oncology (Hem/Onc) will be responsible for providing all aspects of programmatic clinical services for patients on the Hem/Onc service line with emphasis on developing best practices, safety and quality initiatives, therapeutic drug monitoring, clinical pathways, order set maintenance and formulary management. This position is for the day shift with one out of every sixth weekend rotation, to provide comprehensive clinical pharmacy services for Hem/Onc patients to optimize safe, effective and cost-effective drug therapy including:
Multi-disciplinary patient care rounds (daily)
Patient, provider and nursing education
Development of best practice guidelines, safety and quality initiatives and policy related to pharmacotherapy
Expectations of this position are as follows:
Work with multi-disciplinary teams to provide comprehensive pharmacotherapy management for hem/onc patients in the inpatient and the outpatient specialty infusion center
Improve hem/onc services through active participation in quality, safety and cost-savings initiatives, Clinical Management Team meetings, and Pharmacy &Therapeutics Committee meetings
Actively participate in pharmacy intern and residency training programs
Advance oncology pharmacy services through research, education of healthcare professionals, and professional organizational involvement
Have expert knowledge in antineoplastic intravenous admixture preparation including USP 797 & USP 800
Review and refine process for timely and safe antineoplastic preparation and delivery
Pharmacy licensure in the state of Texas and a B.S. Pharm or Pharm. D. degree
Pharmacy Preceptor license in the state of Texas
Completion of an ASHP-accredited PGY2 Pharmacy Residency in Hematology/Oncology; or equivalent experience (PGY1 plus 2 additional years of experience; or 5 years of experience, in health-system pharmacy)
Board Certification in Oncology Pharmacy or eligibility to test
Additional experience in Hematology/Oncology
