Position Responsibilities
Serves as the clinical expert in the area of ambulatory care pharmacy services providing pharmacotherapy recommendations and direct patient care services.
Provides both consultative and direct patient care services with prescriptive authority for referred patients via collaborative practice agreements in the ambulatory care setting.
Orders and interprets pertinent labs and patient assessments for the purposes of monitoring and managing pharmacotherapy regimens of patients to assess efficacy, safety, and toxicity.
Provides in-person and telephonic Medication Therapy Management (MTM) services.
Ongoing participation in multidisciplinary patient care rounds.
Provides timely and effective communication with referring providers and allied health staff regarding patient care activities according to district policy.
Participates in the development and implementation of proactive population management programs for high-risk patient populations.
Documents clinical interventions on an ongoing basis.
Serves as preceptor for pharmacy residents and students.
Serves as faculty for the Family Medicine Residency GME.
Provides didactic lectures and facilitates patient case discussions related to pharmacotherapy to family medicine residents and faculty.

The Inpatient Pharmacy operates 24/7 pharmacy services. A full array of technology is available to assist with distribution of medications to our patients including automated inventory management, automated dispensing cabinets, bar code technology, aut...omated TPN and workflow management systems.
Clinical Inpatient Pharmacy services utilize an integrated decentralized staffing model. Pharmacists practice on nursing units participating in order verification, provision of drug information, and clinical services. Clinical services include pharmacokinetics, anticoagulation management, and automatic intravenous to oral switches, automatic renal dose adjustments and automatic therapeutic interchanges. Specialty pharmacy services include Anemia Management, Anticoagulation, Critical Care, Emergency Medicine, Infectious Disease, Oncology, Pain Management, and Pediatrics.
