SummaryThe individual in this position will be an advanced clinical practitioner having additional expertise in a specific field of pharmacy practice. This individual will lead clinical specialized services offered by the department, having additional training, certification, and/or experience.
Responsibilities- -Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals.
- Deliver medication therapy management within specialized patient care areas and ensure pharmaceutical care programs are appropriately integrated throughout the institution.
- Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to medication therapy management.
- Conduct Medication Use Evaluations (MUEs) and assist in making necessary changes to pharmacotherapy practice as needed based on MUE findings.
Qualifications:
EducationRequired: Pharm. D. or BS in pharmacy ExperienceRequired: 3 years or residency training or equivalent experience Preferred: 2 years in area of specialty CertificationsRequired: Pharmacy License; BLS within 30 days of hire; Preferred: Board Certification in Specialty area, CGP, ACLS, etc. Job: Pharmacist/Staff/Clinical Primary Location: Birmingham, Alabama Facility: Shelby Baptist Med Ctr Job Type: Full-time Shift Type: Days
