The Pharmacy Department at the Columbia Veterans Affairs Medical Center in Columbia, South Carolina is actively recruiting for a Clinical Pharmacy Specialist (CPS) for Acute Care/CLC. This Acute Care/CLC CPS will function at the highest level of clinical practice, working as an advanced practice provider under a scope of practice as defined by the medical center to directly care for patients in the community living center (CLC) and acute care wards as assigned. The incumbent will effectively and efficiently function as a member of an assigned acute care interdisciplinary team, actively participate in rounding with the team daily, and provide medication management services (prescribing and monitoring of anticoagulation, vancomycin/aminoglycoside, etc.). In addition, the CPS will assist the CLC Pharmacist Program Manager and CLC Clinical Pharmacist with medication management (monitoring and prescribing anticoagulation, vancomycin/aminoglycoside, etc.), and will be a backup for assisting with Drug Utilization Reviews when needed. The Acute Care/CLC CPS will promote and manage drug therapy based on current clinical knowledge that is consistent with policies established at the national, VISN, and local levels. The CPS must be capable of using professional judgment and clinical expertise to solve complex problems and to provide clinical information to both providers and Veterans. The CPS must have excellent communication skills and be capable of providing both verbal and written medication information to patients or families on request and appropriate discharge counseling when necessary. This position also requires the ability to perform extensive documentation and clinical progress note writing. Performs other duties as assigned. Work Schedule: Monday - Friday 8:00am-4:30pm. Required to work a rotating weekend and holiday schedule (Tour 8:00am-4:30pm). Tour of duty is subject to change based on needs of agency. Financial Disclosure Report: Not required
