The incumbent will provide vital services to the Emergency Department within the Charlie Norwood VA Medical Center. Duties for this position may include but not limited to: - Operates independently under a defined scope of practice and provides consultative services to include but not limited to anticoagulation, pharmacokinetics, and antimicrobial stewardship. - Develops and manages necessary pharmacy programs and services such as consultation services for acute and chronic disease state management, patient therapeutic drug monitoring, patient and staff education, and comprehensive medication management (CMM) within the emergency department setting. - Recommends drug therapy for veterans based upon pharmaceutical principles of pharmacokinetics and pharmacodynamics and monitor efficacy, side effects, and clinical outcome. - Orders and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the plan of therapy. - Reports and assists as related to drug therapy and preparation in medical emergencies and high-risk procedures such as resuscitations, intubations, acute coronary syndromes, stroke, anticoagulation reversal, toxicologic emergencies, disaster management. - Participates in medication reconciliation admission and discharge process and care coordination to ensure appropriate transitions of care. - Serves as a preceptor to clinical pharmacy students and residents. Participates in the training and orientation of pharmacy students, pharmacy residents, pharmacy staff, and allied health care professionals.
