Ambulatory Pharmacist Clinician The University of Vermont Medical Center - Central Vermont Medical Center Berlin, VT The University of Vermont Medical Center is seeking Ambulatory Pharmacist Clinicians to join theirpractices at Central Vermont Medical Center (CVMC) in Berlin, VT. These positions will work with The University of Vermont Medical Center Specialty Pharmacy team to integrate clinical and operational pharmacy services for specialty pharmacy medications.
The Ambulatory Pharmacist Clinician is an advanced practice pharmacist responsible for assuring optimal drug use outcomes in high risk, high cost or complex patient populations. This position applies specialized knowledge of pharmacotherapy to direct patient care, teaching, and consultation; and participates as a contributing member of ambulatory care teams such as rheumatology, neurology, infectious disease, oncology, nephrology, or primary care. The Clinician provides clinical, pharmacy, and medication expertise in each area including policy development, pharmacy operations, prior authorization support and medication therapy outcomes.
Doctor of Pharmacy and an ASHP accredited residency program or equivalent experience preferred.
Licensed pharmacist in the State of Vermont.
3 years of advanced practice experience in their area of expertise preferred or equivalent experience in ambulatory care or a clinical outpatient setting.
